Service disruption
Dear Patients, to help the Practice cater for the recent GP contractual changes, we are changing our online form provider as of Monday 6th October. This will change the online patient journey slightly where the questionnaire will now be more in depth and follows the NICE guidelines to promote a better triage pathway.
